Application Process
1. Fill out the camper
information form (This is to be filled out by a parent or guardian who knows the camper well. Please provide us with detailed information, it will help us better serve your camper.)
Fax or mail this to Christmount.
If you are applying for a scholarship please send that form as well.
2. You will then receive a camp week assignment by letter.
3. Next you will be asked to send a deposit of $200, set up a payment plan, chose your payment method, and fill out the camper information packet that is sent to you.
4. If you applied for a scholarship you should
hear from us in mid May.
5. You will need to pay your fee in full at least a month before your camper’s scheduled week.

The following forms are to be filled out
once your child has been assigned a camp week
Teacher Form
This is to be filled out by the camper’s teacher so that we can see how your camper functions in a different setting. Encourage the teacher to provide details.
Due: at least a month before camper arrives
Medical Form
This is to be filled out by your child’s doctor. All medications and dosages must be up to date
when the camper arrives in the summer. If there are changes in the medications after you send us the form, we must receive a
written doctor’s note informing us of the change in medication or dosages. All medications (even over-the-counter) must be brought in their
original containers.
Due: at least two weeks before camper arrives
Permission Forms
Photo permission form - we take your campers picture for different reasons, you have the opportunity to
approve of these (i.e. safety, advertising, staff slide show)
Transportation and other permission - allows us to transport your camper, provide medical attention, etc.
