1) We are now requiring a registration deadline. Your
camper
information form and
scholarship
form must be posted by March 16, 2012 to be
considered for enrollment. You are encouraged to apply as soon as possible, as
campers will be
placed in weeks on a rolling admission.
2) Confirmation of your camper's week will be mailed out by March 23rd.
3) Next you will be asked to send a deposit of $200, set up a payment plan, chose your payment method, and fill out the camper information packet that is sent to you.
4) You will need to pay your fee in full before your camper’s scheduled week.

Spring Family Camp Information Sheet
The following forms are to be filled out
once your child has been assigned a camp week
Teacher Form
This is to be filled out by the camper’s teacher or someone who works
closely with your camper so that we can see how your they function in a
different setting. Encourage this person to provide details.
Due: at least two-weeks before camper arrives
Medical Form
This is to be filled out by your child’s doctor. All medications and dosages must be up to date
when the camper arrives in the summer. If there are changes in the medications after you send us the form, we must receive a
written doctor’s note informing us of the change in medication or dosages. All medications (even over-the-counter) must be brought in their
original containers.

Permission Forms
Photo permission form - we take your campers picture for different reasons, you have the opportunity to
approve of these (i.e. safety, advertising, staff slide show)
Transportation and other permission - allows us to transport your camper, provide medical attention, etc.
